Transition comfortably from a hospital stay to your convalescence in the privacy of your own home.

The transition back to normality after leaving the hospital can be just as challenging as the time spent inside the hospital.  Post the hospital stay, the client usually needs additional assistance with activities for an extended period as they convalesce.  Bridges can help make that transition as hassle-free and comfortable as possible.

Depending on the specific needs, it can include:

  • Overnight assistance with ADLs, such as toileting, incontinence, turning, etc.
  • Showering and dressing in the morning
  • Transportation to follow-up doctor’s appointments or physical therapy
  • Meal preparation
  • Medication reminders

Our clients highly prefer home care to a rehabilitation facility, because it allows them to convalesce in the comfort of their own home, instead of extending their away-from-home and away-from-loved-ones experience after a challenging hospital stay.  This service is also great for those clients who would like additional assistance, but do not qualify for rehabilitation facilities.
